Template of Supplement K from the QS9000 APQP manual

Can anyone let me have a template of supplement K from the QS9000 APQP manual?

This is the special charactersitics matrix used along with the Control Plan.

If anyone has this in Excel or Word format, then this would be highly appreciated.

Characteristic traceability is required from the DFMEA through the PFMEA to the Control Plan. Such traceability can be documented on the "Supplement K" available on https://web.qpr.ford.com/sta/Phased_PPAP.html.

Above is the requirement from the "Ford specifics for use with PPAP 4.0".

Can anyone give the template of the "Supplement K" document ?

I have no possibility to login at Ford's https but i don't think it is copyrighted material.
