I Say... What is FRACAS? Failure Reporting Analysis and Corrective Action System

What is FRACAS? Is it the same as a CAR? I have been asked to write a procedure and I have no ideal what to write anyone have an example I can look at?

Failure Reporting and Corrective Action System (Think I got that right)

I am most familiar with it from Reliability & Maintainability and maintenance stuff. I went through Ford R&M - not sure if it is just a Ford term or not.

It is basically a corrective action for a equipment failure.

Failure Reporting and Corrective Action System? I vaguely remember hearing of this before as a CAR-type system for capturing and addressing field failures. Maybe with missile systems?
