Thank You! Management Review Procedure / Form - Needed

Can anyone provide a copy of their management review procedure? I'm trying to figure out the best way to handle/rewrite our management review meetings. We have management reviews bi-annually with all the required inputs/outputs but our auditor stated that we needed to show that we also review items as they arise i.e. customer satisfaction, CAR's, and process issues.

Management Review: Separate Procedure or Description in Quality Manual?

Here's our stuff. We don't have a level II procedure - just the attached excerpt from our QPM. The minutes attached are for our Annual Year End Management Review; I'll "edit" and post the minutes from our last Quarterly Management Review as soon as I can.

I had a minute sooner than I thought. The meeting memo is sent to everyone ahead of time so they know the agenda (black font only) and I add the minutes in blue once the meeting has been conducted.

Hope this is helpful. Where you see "..." is where I removed our customers' names.
