Example(s) of simple CAPA (Corrective and Preventive Action) forms
I am new to this forum (glad I found it!) as well as being new to my company. We are currently using a complex CAPA form supplied by one of our customers. I am trying to design a simple form for our smallish machine shop (20+) that will work. I would appreciate any ideas and/or examples. Thank you.

Hi IamDory and Welcome to the Cove ,
Along the top bar of each page is a link to the search function. Click on it and go to advance search. Type in "CAPA" and you will find threads on this subject with attachments that will contain examples of what you may need.
Good Luck and Happy Hunting!

As Carol said welcome.
I have attached copies of CA and PA forms that I use in a 45 person Machine Shop. They may or may not work for your situation.

Re: Seeking example(s) of simple CAPA (Corrective and Preventive Action) forms
Coury,
I liked the simple and to the point attachments. I am in the process of writing the nonconformance procedure and trying to make eveything all fit together. As you can tell, I am very new to the QA world and need lots of help. Any idea on procedures for nonconformance, corrective and preventive procedures. Did you combine corrective and preventive or did you do each one as separate?

Please see attached a simple corrective/preventive action procedure I used with a company for a joint ISO 9001/14001 certification. Bear in mind that it was a very small company and it may not suit a larger one.

Colin, thank you so much, we are a company of 15 just getting going. We have become involved in the European market and ISO and the CE are required. Thanks.

The issue of one or two procedures was discussed in detail here. Or, under the Search button, try " Corrective Action one procedure " and you'll find lots of related threads.
The concepts of nonconforming product (and it's control) as well as corrective and preventive actions are probably among the most discussed concepts in the forum...and you may not find one clear answer.
Some organizations do the bare minimum...some go beyond.
What I would keep in mind is to ensure the system meets your organization's needs. Don't have your organization assume funky yoga positions to meet the requirements of the standard...make the standard conform to your organization!
