Please Help! RPN reduction - Occurrance Reduction - Reducing occurance of defect on line

Please give me some good exampole of RPN reduction, specially Occurance Reduction.

In short i will like to know how we can reduce occurance of defect on line e.g By Errorproofing (I need such more example with some explanation)

You need to be more specific. What are you looking to accomplish. What is your current situation?
