Please Help! RPN reduction - Occurrance Reduction - Reducing occurance of defect on line

Please give me some good exampole of RPN reduction, specially Occurance Reduction.

In short i will like to know how we can reduce occurance of defect on line e.g By Errorproofing (I need such more example with some explanation)
