Please Help! Formulas and Constants - Control Limits for Individual and Moving Range Control Chart

Can someone supply me the formulas and the constants for calculating the upper and lower control limits for an individual control chart and the moving range control chart. We are using a moving range based on two samples.

As always, your assistance is appreciated.

Altho the constant 2.66 = 3/d2 = 3/1.128
