Investigation Report Template - Needed

I am looking for an investigation template to use for quality related defect investigations. We currently have nothing in place and it will be quicker to amend one rather than create afresh.

We just started work on a "containment" project - maybe this will help. But please remember - this has been tailored to my company and there are terms on this form that will mean nothing to you.

Here is something we are working on. I have attached a flowchart and an Investigation form, to be followed during the investigation. We are still reviewing this process but it has worked really well with safety (there may still be safety referencees in the process at this stage). Hope it helps
