FMEA Information Centre Internet Web Site and FMEA Use Survey
For your perusal
"Hello,
We kindly request your help to collect data for our study about the use and spread of FMEA - Failure Mode and Effects Analysis as a major quality tool.
Could you therefore please:
1. Complete the survey on *** Dead Link removed *** (10 short questions), it takes less than 2 minutes to complete the questionnaire). In return for your help we will make the results of this study available to all (some results of a previous survey are already available).
2. Forward this mail to 5 (or more) people in other organisations. We hope to get 1.000 or more completed surveys by end of January 2000, this is of course only possible with your help. We will send a bottle of champagne to number 1000 ;-)
Many thanks,
The Fmea Survey Team.

Survey's long gone, but the site is still up! Just an FYI, folks.

I'm curious as to who got the champagne. Was the brand Dom Perignon?

I don't remember hearing anything about the outcome of the survey. I came across this as I was going through old threads continuing to weed out dead links.
