Please Help! Manufacturing Process Audits using "Football" diagram

There was a very interesting format for Manufacturing Process Audits posted some time ago, in which a "Manufootball" was used, sort of a mutated turtle diagram.
I'm not very good at navigating around the cove , and I can not seem to locate who posted this, so I could ask if a corresponding procedure could be made available. Can you help me locate the originator?

Thanks!
