I am looking for a form (or questionnaire)that is adequate to send to a supplier (or sub-contractor) in order that he may indicate the degree of his compliance with ISO 9000. I plan to use this form in lieu of a supplier audit. Can anybody advise me on the necessary contents of the form or the advisability of the practice.
adviseability of supplier surveys? Well thats quite a debate......to me it defaults to the commodity to be purchased, and if its a value added exercise...it is a 'way out' to many, in lieu of any other creative thoughts.
What should be on a survey? WEll...typically I see (right or wrong) Are you compliant/registered to a quality system? which one? if yes, sign this survey, and attach a copy of your registration....yada yada yada.
THEN they go on either in summary or detail...asking the ISO 9001 shalls...with a scale to rate the comnpany status...1-5...1 being no system, 5 being a documented system with records. The best ones allow a phone interview to provide the answers so as not to hold up a purchase...as well as mail in parallel and also can be used as a site verification record.
Others, are actually written around contractual requirements...the question always comes back to...what do you NEED/WANT to know?
Whatever you do, allow for a review and disposition by your company.

Why? Is it not of more value to establish if they can supply you with goods or services that meet your requirements?
What is this fascination with supplier questionnaires? I have to agree with Barb, it is a way out. Think about it. The only important issue is supplier performance. Can they consistently supply to your requirements. Measure that. Everything else - questionnaires, even supplier audits is wasted effort and a drain on resources.
How much time do you spend having to fill in questionnaires from your customers and how much time do they spend assessing that information? Its only 'value' is when retrieved from a cabinet to show the ISO assessor. There happy, you breath a sigh of releif. Forget it. Approve everyone and measure performance. Can they meet your requirement? As far as 4.6.2 goes thats all that matters.
Do you have copies of survey or audits from your customers. If you do, use them as a guidline to develope your own. This usually works better than trying to find a "canned" form that you would need to modify anyway.
