I have been at it again! Apologies to all the Covers from outside the UK but some of the terminology is national. The HMRC is our equivalent of the IRS (plus some) - they managed to lose a couple of data discs with the bank records of 25 million UK subjects. I was asked to write an article for CQI's Quality World magazine about the problem and investigation. It is available freely to non members here.
