Is it required to complete this?

Stijloor, I am a bit out of my depth with OHS but I think the employer is required to complete the form following an accident falling under the Reporting of Injuries, Diseases and Dangerous Occurrence Regulations (RIDDOR) 1995 see the HSE website for further (proper  ) guidance on the matter.

Don't know if this is helpful, but here goes.
Initially, a database entry is made with:
Date and time occurred:
Incident number assigned to this case:
Supervisor:
Specific location:
Detailed Description
Preliminary Cause
Type of Incident:
Immediate Corrective Action (Completed: No)
Then, the incident is reviewed by our safety committee and cause determined (or verified) and long term actions are determined (counseling, something needs to be fixed, procedures need to be put into place)
Also, at the time the incident is reported there is a drug test performed. Failure to report an incident is cause for dismissal.
