Please Help! Life Cycle Cost worksheet - Reliability and Maintainability of systems

Good day.

This is my first discussion! :0)

I am a quality manager with 20+ years experience in a number of manufacturing processes and materials. I hope I can help when needed.
I am beginning to get back in to Reliability and Maintainability of systems.

I am looking for a LCC worksheet such as the one GM uses. Any other effective tracking worksheets will be helpful.

This is not really my area of specialty. I did a search, and did not really locate anything. Maybe someone else might have something for you.

I found a thread on KPI and some other stuff. Don't know if it is helpful:
