Regarding one company that we worked for recently, we developed the control of non-conforming products and the corrective action together, and on the other hand we had a procedure for the preventive action by itself. However, we were caught by the external auditor, that we did not have the process to initiate the preventive action.

This is the form we use, some explaination of the Acronyms, I work in a Military Defence enviroment so we have our own language.
TAW-Technical Airworthiness (Our regulatory body)
QM=Quality Manual
AS9100 - The standard we are registered to.
SER=Supplier Exception Report - used identify issues with suppliers.
DEV/WAV- Deviation or Waiver requests from us to the Customer.
PIF=Preinstallation Failure (initiates the TIR from our Customer)
TIR=Technical Investigation Report
EIR=Engineering Investigation Report
CBEA=Complete before end of Audit/Investigation
OFI=Opportunity for Improvement
IA=Immediate Corrective Action
PA=Permanant Corrective Action
MRB=Material Review Board

This what we use now, the PA blocks are to be use when we are looking forward during process audits and see a potential for failure. The bulk of our work is done in the CA blocks. All areas of the form can grow. The form has been sanatized. I just felt like sharing with everyone. This is my first post, all comments are welcome.
