Recently , I received several survey from my customers regarding REACH.
1. In one survey , there are 16 chemicals which the customer required us to indicate whether any of these chemicals are present in the material and the % content.
2. For another customer , the survey form indicate only 5 chemicals to declare.
3. For a 3rd customer , they provide a so call REACH chemical list containing about 270 chemicals for the survey.
I am rather confused by all these different chemical list. Is there a standard REACH chemical list??
I need to get my material suppliers to do a similar survey. Which chemical list do I need to provide for my suppliers

It will be a confusing situation for awhile (several years, I would think). Your customers are trying to do the best they can with the information they have.
You have to pass on the "ever-changing" requirements from your customer(s) to your suppliers.

It is possible that your customers are making it easy for you to prepare for REACH compliance. Instead of giving the list of all the concerned chemical substances, they are perhaps asking you about substances that are likely to be present in the products that you supply. For example, a metal part or a glass part will not have the organic substances listed (unless specifically coated); there is no point is including these organic substances in the list sent to you if you supply metal or glass. Similar considerations from different customers result in different numbers of chemical substances in your products about which they need information. A customer who is not sure sends the whole list to you. That is why you see these different numbers.
I also understand that beside the "candidate list" , there is another Annex XIV List. What is the different between the 2 list?? Which list should I follow for SVHC??
