Please Help! Subcontractor Assessment - Change control process

We receieved a Subcontractors assessment in which one item is puzzling us. Perhaps someone could shed some light on this for me.

Er, have to talked to the people who sent it to you? It's a little difficult trying to second guess what someone is after, asking these kinds of questions.....

I agree. It's not a clear question at all! And without your providing any background or context (just the request), it would/could take a long time and a whole lot of guessing and still could be wrong.
