Please Help! Certificate of Compliance-looking for template and have questions

I am looking for help, I have a request from a customer (automotive) that would like a Certificate of Compliance for a few part numbers, the product is Plastic Injection parts if there is a difference. I was wondering if there is a templete or required Information that needs to be on it.

if specs weren't called out might want to say something along the lines of 'requirements met per the purchase order'.
