Root Cause
Has anyone here ever wanted fill out stupidity as the Root Cause for a Non-conformance. When the same error occurs time and time again what else can you say. I know that is not a valid root cause, but the situation will never go away unless the person leaves by their own choice. What are some of the other things others out there would like to be able to put down as the root cause. I could use a good laugh before our Recert. Audit in 15 days.

More accurate would be lack of Mgmt Support/Enforcement for a 'lasting' CA solution.

In the document section, I think, there is a thread called "I wrote the procedure and I goofed". It's full of suggestions and discussion on the very same thing. I printed out one section for possible use in the future.

I think that at least 5 times per year, one should be allowed to use, "Stupid" and/or "Usage of Bad Crack" as an excuse. I totally hear you on this one!! It just seems like that same freakin' people continue to make dumb mistakes.
I take the NCR to them and ask them how this happened. They just sort of shrug their shoulders and tell me this things happen. I just want to smack them upside the head- HARD! If I had the power I'd can their sorry little asses. But I don't, so I'll just fantasize about it here.

Fire Girl I love your response. I wish I didnt share an office with someone because I would be rolling all ove the floor laughing my butt off. I have no problem with the excuse stuff happens cause it does we all have bad days etc... but when it happens over and over and worst of all this person is one of our Quality Managers ugh!! I sit back and laugh to myself and just thank my lucky stars that i am not that stupid. I look forward to our Recert Audit it is the Department managers that don't. It will be a wonderful three days. At least I get luch for 3 days out of the deal.

Operator taken out back door and beaten soundly about the head and neck with a rubber hose. Hands not touched because he will be transferred to Janitorial Department.
Long Term Corrective Action:

Investment in larger gauge rubber hose, increase in health care co-pays, and additional funding for Janitorial Department.

The a$$hole was fire (how much more corrective or long-term can ya get?)

HR Manager will be summarily executed if he ever interviews this individual again.
