Please Help! 8D Report format and the best and most effective method of report preparation

Can any one suggest me the best and effective method of 8d report preperation and provide the format for the same.

presently we are doing with a simple format where different 8 dicipline is mentioned under which we use some tools like why-why analysis,fishbone and pareto for finding out main rootcause.

simply suggest me how to close the loop of complaint from start to end.
