One of the important things of doing HACCP is to go through that process first, looking at the products you produce in a very structured fashion, for each product identifying the risks and how you will control them.
After you've done that and know what your risks are, THEN think about what procedures or flowcharts you need. I doubt anyone could just 'send an index' at this point - I certainly couldn't.
