Hi Ana:

I've adopted a very similar supplier rating system (probably gotten off the Cove too long ago to remember ) to what Sam posted above but minus the "Supplier Field Returns" rating category. However, I use this for only my Supplier Monitoring. I use a different "Risk Based" supplier qualification method when we take on a new supplier or purchase a new custom part from an existing supplier.

We also send out a Survey and Include things like the Survey Results, Audit Report and ISO Cert's into this file as separate worksheets so that the supplier qualification is a "one stop shop" to review our suppliers' qualifications.
