NCR (Nonconformance Report) Combined with Corrective Action Report?

Working to get the records control completed, just wondering if it is acceptable to have combine Nonconformance Report Form and the corresponding corrective/preventive action on one record. Would seem inefficient if they were to be seperated but im not sure what ISO demands
