Default Oops - Sample Survey not Sanitized

Neal (et.al.),

I had to delete the previous sample since I had not fully sanitized the survey (missed taking the company's name out). The following attachment has been fully sanitized.
Attached Files: 1. Scan for viruses before using, 2. Please report any 'bad' files by Reporting the post it is in, 3. Use at your Own Risk.
File Type: doc a customer satisfaction survey (091101).doc (54.0 KB, 1172 views)
