I Say... Quality System Ineffective - Fishbone Diagram the QMS System

Greetings Quality Geeks around the world ! A slow day has drifted me into looking at some old techniques and today’s flavor is Fishbone. After looking at some old and very useful diagrams, I began to think that none of them seem to address what in my opinion is a Global Problem Statement: Quality System Ineffectiveness. So, after some more thought, another coffee and drawing conclusions on lessons learned, I came up with some Bones that may stimulate some thoughts on where we need to go in order to improve declining systems. Because of wretched commitment and an inept registration system, unless something drastic happens, I don’t see myself in Quality much longer. But as long as I am, I will try and offer solutions, no matter how odd they may appear. And, if they stimulate one tiny improvement, it was well worth it. Have a day !

Sorry to hear about (read about?) your frustration. I suppose that we all have felt our share of the same, but I commend your attempt to put a positive spin on things. Being in Quality is not for the faint of heart.

I'd have to start with Leadership as I believe that the faiure of a QMS (or business system) is born out of and inability to lead (there are other factors of course). Being at the head of an organization doesn't automatically make one a leader. And, even if what you find at the top is a leader, there aren't any gaurantees that this leader stands for something positive. But in regards QMS ineffectiveness, I put the responsibility of failure (or success) with leadership and then management.

What is the QMS about? What does it stand for? What is the Vision? So, for a bone or two, I suggest:

-Lack of Vision
-Lack of Commitment to the Vision
-Failure to create Commitment (Compliance is easy if you use fear or incentives, at least initially, but commitment takes longterm understanding)
-Wrong Leadership Style (i.e. Destructor, Procrastinator, Caretaker as defined by Crosby)

Well, here is a start.

1st of all...BOX!! Can I use this thing in some of my course presentations? I'll give you credit of course. (But no money)


2nd....Kevin, you're sounding like me here. You'd better watch out or your rating will slip into oblivian like mine. I can do better though!

As far as I'm concerned most of the 'leaders" we see aren't. These folks (boys and girls, but mainly the boys) are manipulators that have climbed the rungs of success on the backs of others. These "leaders' are micro-managing, power poisoned, Tunopia stricken people that 1st and foremost put themselves at the top of the priority list. Without wanting to offend anyone, most of this crowd's parking spots should be in the handicapped area because they apparently can't hear, see or even feel, evidence bears this out. Takers?

3rd......most system management is done as a catch-as-catch-can type of operation. These things weren't built with value in them, therefore no real value is expected from them. These things are overhead, an expense, something that gets in the way of our doing business. Am I off traget here?

Well said as always Kevin. Frustration will soon switch to anger and then insanity if I think Quality will ever again be held in the highest regard. It's almost turned into a burden. But, the occasional consulting gig will be fun, as those companies are humble and truly want to improve. Too bad they are the exception !!
