Question PPAP Version 3

Has anyone heard of any suppliers or manufacturers having problems due to PPAP Version 3 that went into effect last week? Just wondering, from my perspective, I thought lots of companies would be caught due to this change...If you know anything, please let me know! Thanks

I have a question on the new form CFG-1001. Can anyone tell me what Mold/Cavity/Production Process means? It is a line item on the form and we are submitting PPAP with this form and our customer doesn't even know what it means (Tier 1). The only explanation in the manual is to complete it. Also, "I further warrant these samples were produced at the production rate of _________/8 hours. I have noted any deviations from this declaration below." Any suggestions?

Mould/cavity/process/ refers to M/C plastic parts or metal stamping etc, how many cavities are produced . Process is the process used. I have also been asked to put the % of regrind used in this place.
8 hours.... refers to the significant production run, see 1.2.1.
