Please Help! Converting to Process Audits - Seeking: Process Audit Form layout

I am in process to update our audit paperwork to a process style. My company was ISO established 9 months ago with a checklist style of form.
I recently came on board and now that our new cycle of audits are about to begin, i want to convert to Process formatting to better align with our needs. However, not sure as to how to set up the documnet.
Any feedback is welcomed.

This example is for just 1 process. But you may use the concept to extend to other processes.
There could many examples,if you did a search in Cove.

You might also look through the auditing board. Lots of great ideas there.

Attached is the process audit form we use, kinda of a turtle laying on his side This format is the same as our registrar uses.
