Seeking: Subcontractor Satisfaction questionnaire - Construction Industry
Dear all expertises
I am seeking for the questionnaire for asking the subcontractor and client in the safety satisfaction in construction.
if not applicable, i just want to ask your ideas that what should i ask in the questionnaire?

Welcome to the Cove
It may take a day or two to get some answers - this weekend is the last big holiday of summer in the USA and most folk won't be back to their computers until Tuesday.
Why not tell us a few more details about your situation - things like - type of project (residential, commercial, office, industrial?)
- size of project? (estimated cost or square feet/meters?)
- your position in the situation (general contractor? architect?)
- are these safety issues to be part of the requirements for bidding by subcontractors?
- timetable for the project (has everyone agreed on blueprints? Is the financing secured? land purchased?)
- are there any governmental regulations that have to be considered?
- are all building permits from government authorities already issued?
- is there liability insurance on the project?
I could give you any one of a number of "due diligence" checklists for new construction, but some are specialized for certain types of construction and all are specialized for geographic location (within a political division of land - town, county, state, country - which all have slightly different laws and rules to consider.) In any regard, if you are not already somewhat knowledgeable and experienced in the field of construction management, they could be misleading and lead you astray to considering more or less than really required - either route being excess expense without value return.

The situation is like this. I am a main contractor, which have forced my subcontractors to apply my safety program in the buiding contruction. The issue is simply, after all subcontractors applied my safety program, i just want to survey the feedback that what do they encounter with this saety program such as cost incurreed, difficulty for working and so on.
Therefore, i would like "expertises" here help me to establish the question the gather their satisfaction with this safety program. That's all.
