Please Help! Management Review Checklist / Form / Procedure - Seeking Examples

I had a form somewhere I liked.. but apparently have lost it now.. (surprised I even know where my head is).. anyway.. do you use a management review check off sheet.. or some other sort of one page symbol of their attendance?..

We have a set program for our management review (Quality Steering Committee here), with the list of items we review each time. Every team member has a review book, with this listing in it. Each person reports on their area of responsibility (ies) as it gets to their turn. Minutes are produced of the meeting for the entire plant, which includes a roll call of the committee. We don't meet unless 90% of the team can make it, and the boss usually ensures this will happen.
