Are the different pages in the PPAP (control plan, PFEMA, PSW, etc..) supposed to be controlled documents?

If you use the templates provided as part of your (IATF) management system, then yes, they must be controlled.

It's not clear whether you're asking about forms in the manual (the subject of this thread) or copies of actual PPAP submissions to customers. Please explain further.
