Completing 8D in the section "System Prevention Action To Prevent Recurrence"

I need help...I am flooded in corrective actions due to a disgruntle employee..unfortunately he was the process tech and yes I said was, A new process tech starts monday. (Hurray) Now to complete the mutli - corrective actions for short shots. Does anyone have any creatIve ideas how to complete the 8D in the section "System Prevention Action To Prevent Recurrence"- other then a QC Audit to each carton prior to shipping?.

QC audit of every box does not address this section.

You have to analyze your management system. How was the old process tech able to what he did? What other processes are exposed to the same issues? How can one process tech create such havoc? What kind of controls does your company need to prevent?

Something to think about:

- error proofing
- layered process audit
- management involvement
- automation
- shift responsibility away from tech and to operator

You also must address (maybe not in the 8D) why you have disgruntled employees. Usually management is at fault, not the employee. Look at your hiring practices, are you hiring the right people. If one have disgruntled employee, there are potentially others.

This is not just a quality problem. You must get your management involved. If they do not want to you, you know where the problem lies.

More information, please. How did a disgruntled employee cause a lot of short shots? What's the connection? What's a process tech? You're asking for help in determining preventive action but you haven't told us what happened.

Yes, its a management problems, due to lack of work and layoff the responsible person created havoc in the process area to look as if there was a need to his assistance. anyway, thanks for your helpful input always nice to have other ideas when your at wits end.

Thank you so much Ajit. Unfortunately I will be writing up the 8D solo I used to work for a much larger company with high tech vision inspection w/poke yokes for all the tools, currently the company does not have the capabilities to implement poke yokes or the time to 100% sort everything, most of the parts are the size of peas and multiple cavities, its going to be a challenge but hopefully with a new process tech that has a background in process engineering he can help us exceed the customer expectations.
