System FMEA, ever do that? and how?

Often focus is on DFMEA and PFMEA. Prior to these there is the SystemFMEA, though.
Wonder if anyone actually do SFMEA's, focus on this topic is rather vague in the QS manual as well.
I tried a couple of times to produce a SFMEA. My approach was to list costumer requirements one by one, and then identify possible sub-system causes for the systems failure mode and rank them accordingly. In this way I could already identify somecritical and non-critical requirements, before the next step (DFMEA-PFMEA).
I find it difficult to steer clear of a detailed DFMEA during this stage and therefore get a "muddy" transition to the DFMEA.
Anyone who has input on difference and handling of SFMEA versus DFMEA?

We apply the SFMEA in the entrance of the project (all inputs), the systems of data entries (all inputs), is very similar 5º phase of the APQP that also almost nobody applies, elaborates as if it was a TGR and TGW in its sistems and works in top of them to elaborate a SFMEA.

In case that it has doubt in the application of exactly, it can be the will to clarify its doubts.

I have to ask though.... why is the SEV ranking only a 3 through all 3 FMEAs when the Effect is "Extreme physical discomfort and customer dissatisfaction"?

The 4 was reserved for out and out non-compliance and severe hazard issues.

This is an example of a reduced scale for simplicities sake. Not all products /processes may fit the 10 point scaling referenced in the AIAG manual.

There were 4's within the FMEA's.
