Should "missed process step" be filled in PFMEA?

Hello all:
There is an interesting thing.
If there is a production line with below process steps:OP10,OP20,OP30 ,OP40. The product should flow step-by-step ,of course there is a possible that the product miss one or two steps such as it is delivered to OP30 when it just finished OP10,we can see that it miss the step2(OP20),then maybe bring defect.

So should this case be filled in PFMEA?

Yes. That's the time to look at the possibility of missing a step and do something to prevent it.

Yes, even if it is a continuous flow line.

Parts can be taken from the line (extended down time for a machine down stream, for example), put in a box, moved into storage, brought back, put back on the line later. What are the risks putting the parts back on the line in the wrong location? (this could be disasterous for a machining centre).
