In my opinion, if a decent root cause investigation leads to that, it would be acceptable to use operator error as a root cause. After all, people will make mistakes from time to time. Most knowledgeable people frown upon the abuse and misuse of "operator error" as a common cause for defects. If you have a lot of non-conforming products, due to operator error, your system is lacking in terms of evaluating operator competence.
Additionally, if it was an honest "operator error", you have to question the validity of re-training as corrective action and explore the possibility of mistake proofing that operation by other means.
