What document to use to document Welding PV testing - Supplier to Ford

Is there a form for PV testing on product supplier to Ford?

This is testing for welding, that apears in the weld specification which is stated on the print.

Could the DVP&R form be used if we are not design responsible?

I know I could ask the customer, but I want to hear what the group has to say.
