Examples of Nonconformance, Corrective Action Requests, and Root Cause Analysis

We are an ASME Pressure Vessel manufacturing and repair service company in the petrochemical arena. Recently we have decided to improve our QMS to include a formal NCR, CAR and Root Cause analysis program.

MY back ground is in the field installation but have had some limited training in the Lean, 5S, NCR and CAR systems.

My deadline is coming up very quickly to get the NEW system outlined with an NCR / CAR procedure complete with document / form templates.

This is where I lack the experience using word, excel, and other tools for standardized documents and forms.

Searching around the forum and threads I have come across a few sample forms and documents but still not finding what would suit us best.

This will be the initial start of a QMS program that will develop into a well suited NCR, CAR and Root Cause System headed for ISO 9000 acceptance.

Many of us including me are new to this and learning as we go. So, something that is at a pharmaceutical rating is just too much to start with right now.

If there is anyone willing to assist or direct me to a usable set of simple forms and documents for the initial setting up of a NCR / CAR program it would be greatly appreciated.

My knowledge and understanding of the NCR / CAR system is pretty solid for getting solid but finding the appropriate forms or creating them is proving to be extremely challenging to me.

Direction to a simplified software for purchase would be helpful. My primary search is for the editable forms and report documents.

You may want to take a look at Bugzilla. It is open source software. We have used it for ten years for handling non-conformities and corrective actions. Their forms are easily adaptable to these applications. Reporting and reminders can be automated. We've been quite happy with it.
