I am used to working with both of the standards. Where I work now we only have a liscence for API Q1. My question is do QC inspectors need an anual eye exam according to API Q1 or is that an ISO requirement?

I can name one example of a standard that does require it: EN 473:2008, Non-destructive Testing - Qualification and certification of NDT Personnel - General principles.
Clause 6.4 - Vision requirements - All levels:
The candidate shall provide documentary evidence of satisfactory vision in accordance with the following requirements:
a) near vision acuity shall permit reading a minimum of Jaeger number 1 or Times Roman N 4.5 or equivalent letters (having a height of 1.6 mm) at not less than 30 centimeters with one or both eyes, either corrected or uncorrected.
b) colour vision shall be sufficient that the candidate can distinguish and differentiate between the colours or shades of gray used in the NDT method concerned as specified by the employer.
The documented tests of visual acuity according to 6.4a) shall be carried out at least annually.
Now... As quite a few of our customers require us to adhere to EN 473, this requirement has been added to our management system. I expect that many other producers have the same requirement in their written procedures, and it is quite likely that some believe it to be a requirement in ISO 9001:2008, which it is not. However, in our case 7.1 & 7.2 comes into play.
