First off you have to become a member of GMDN agency to get the codes(legal route!). If you are a manufacturer then there is a sliding scale. Go to:

http://www.gmdnagency.com/Info.aspx?pageid=6

The minimum fee is 200 euros for 5 codes if you are a small manufacturer. The class of device is irrelevant.
