Please Help! Clarification on Clinical Evaluations and Clinical Evaluation Reports

Dear friends,
I need a small clarification on Clinical evaluation and clinical evaluation report.
These two items are same or is there any difference between the two items.
Could you please have a look on this and reply me with simple clarity.

Yes they are different, but you can combine them into one document.
Basically the clinical evaluation will cover the clinical benefits and a comparison against other devices already on the market. The report will summarise your findings and can include the evaluation.
There are other requirements as well, but I think I have answered your question with the simplicity you wanted.

Clinical evaluation = Activity.

and

clinical evaluation report = Record of activity.
