How to develop a strategy to eliminate an indicator who has been 0 forever?

I am since March Quality Manager. I have to develop a strategy to eliminate an indicator who has been 0 forever : falls in operating.

I produce steering elements for automotive and is hard to have o fall in operating, because that means that can even die.

I need actions to achieve 0 falls in operating. Does anybody has an idea ?

What you probably need is a "leading indicator". Just as companies want to have the number of deaths on the job be zero, the only way to keep it at zero is to monitor things you do to prevent deaths, things that "could have been a death" (close calls), or things correlated with deaths.

I don't quite know what a "fall in operating" is, but I assume it is some form of rare but severe event (sentinel event). Take a look to see if you develop supporting indicators for close calls, preventive actions, and correlated indicators.
