Change Control Procedure and form / template - TS 16949 Clause 7.1.4

Hello,
My company is Tier-1 supplier (Tyre Wheel Assembly business). We are a very small company (26 people). We don't have design function and we've been already certified for ISO/TS16949:2002.
1. I'm very confused about change control (7.1.4) and APQP. Could anyone explain the main differences to me please?
2. Could you please attach an example procedure and form for change control process? A very simple and effective process is more appreciable to suit to our small business.

APQP is a linear process. Design or process change control is a part of that process. When a change occurs you essentialy loop back to the appropriate point in the APQP process and proceed from there.
