Transfer of Product to Production - EPC/Safe Launch/GP12 and VDA 6.3

Our company recently went through a VDA 6.3 audit and one of the findings was in the aera of transfer of product to production controlled to secure product launch. We currently have 4 parts that are being process through EPC/Safe Launch/GP12...what ever term you are use to seeing, it all means the same.

What I am looking for an example of a communication document or form to communicate between Engineering and Manufacturing that the part is exiting EPC/Safe Launch/GP12.

Any help will be greatly appreciated.

Here an example from my previous position as QM.
2 of the 4 plants I was responsible for were mainly dedicated to German OEM and the processes where audited regularly to the VDA 6.3 standard.
