Looking for Document Control Templates

Hello friends,

I am urgently looking for the following templates:

1. Document Change Request (DCR)
2. DCO Log to give a unique number to the DCRs.
3. Document Transmittal Note to issue new procedure and retrieve old copy.

I searched the attachment list but did not find one that I could adopt.
