Should the Line Clearance include Machine Setup for current Operation?

usually, line clearance is carried out by QA, to ensure that all the previous steps are cleared (avoid mix up and contamination etc.,);
then the machine setup (includes mounting new product specific change parts etc, completing the initial runs to ensure desired performance) are taken care by production supervisor.

the question is, should this initial machine setup be part of QA - line clearance? (should QA be part of initial-machine setup?, especially for critical operations, which determine the product outcome/performance)

The question is about the process and who has all the authority and competency to perform this.
Let us not take and make QA a policeman. If it is usually performed by QA as you say, then the only question is usually where ?
If that is the practice in your company, and the personnel in QA are both authorized and competent, it is fine.

The production supervisor if at your place is competent, authorize him. If a QA inspector is competent in the inspection aspect, he can certainly assist the process and be involved in the team.

As a side note along the same lines, don’t overlook Maintenance when it comes to Line Clearance. Unsticking a part, disengaging an interlock to perform a repair, cycling through a couple parts after the fix to make sure it works again, changing over a unit that required calibration or re-mastering etc..
Maintenance may not be responsible for the part quality (know what checks are required) but they should be responsible that any parts they cycle or pull out of a stuck machine are properly segregated and held for the folks who do.
