Donabedian Model in Healthcare Setup

Hi,
I want to know about donabedian model in healthcare setup.
What I understood is that it is a model based on structure, process and outcomes.
Structure gaps are related with environment in which the services are provided.
Process gaps are related with process in health care delivery system. delivered.
Outcome gaps are related with the results of the processes in the health care setup.
Please correct me if I understood wrong.

Now, I want to know for ex. if there is some equipment present in the hospital but it is not functional then whether this will come under structure or process gap.

Also let me know where to look for.

I just viewed some information with regard to Donabedian Model and in my understanding, it deals with the Patient Safety Management System.
I used google and find out this path - As a reference: http://www.ahrq.gov/research/findings/final-reports/medteam/figure2.html.
Regarding your query, if the said equipment were not functional it will definitely fall under the antecedent conditions. Said model looks like the same with process approach (turtle diagram), that you have the necessary inputs.
