We call NCR's MDR's which lead to RCA's. Corporate has streamlined processes to use one form for Supplier and Internal Corrective Actions, as well as Preventive Actions:

(The fields are meant to expand when data is added. Place your company logo in top left block, use whatever identifier you wish in the footer/header with revision info if your system is so set up.)

If possible I would like to see more examples.
