What can ideally be the contents of a document called 'Past trouble data base'? The document/database is intended to capitalise on the past troubles such as Customer concern, Warranty concern, Internal concern, Supplier concern, etc for new product or process.

The idea of a past trouble database is so that for future projects you have a record of any potential problems which can be fed into design of future product/process to avoid making the same mistakes again.

Issues to be recorded could include problems with design, equipment, manufacturing processes, customer concerns, warranty, internal concerns, scrap, rework, material handling, etc.

This information can then be considered when preparing Design FMEA, Process FMEA and identification and control of special characteristics for new similar projects.

The idea of a past trouble database is so that for future projects you have a record of any potential problems which can be fed into design of future product/process to avoid making the same mistakes again.

For our Automotive Supplier Covers: The AIAG APQP Manual 2nd Edition calls it "Lessons Learned." An excellent and often overlooked tool. Have the IT people help you with setting one (database) up. I look at this as an important organizational asset that can not be purchased. Yes, Lessons Learned. The tuition has already been paid. You don't want to pay twice.

There is not really a "standard" format. The best thing is to determine (with the help of a database management competent person) what quality management systems "lessons learned" information you want to maintain and how you want to manage it. Yes, it takes work to set this up. Maybe other Covers can provide you with an example.
