Looking for Chrysler FPSC form per procedure PSSP0106 - From Chrysler 2001 PSO manual

Hi- our company is pretty new to using powerway.com. One of the PPAP requirements is the FPSC form. I know what FPSC is, but is there a specific form anywhere? I've searched the PSO 5th edition and I can't find it. If anyone can point me in the right direction I'd appreciate it. Thanks.

This is from a Chrysler PSO manual I have that's dated 2001 (I have no idea whether it's up to date or not):

DaimlerChrysler FPSC blank forms along with instructions for their completion per procedure PSSP0106 may be found in the DaimlerChrysler Extended Enterprise™ Network, through PRISM on the Extended Enterprise​
