Try looking at this for the portion for developing the chart:

The CORRECT steps to implement an SPC chart

As far as the rules and reaction plan, that depends on the chart you end up using. The reaction plan for the rule should address the specific action it takes to get the process back in control (adjustment, repair, replace, etc.)

Is there a process flow chart, PFMEA, or a control plan? If so the WIs will impact them...
