Supplier Assessment Surveys - Where I can see some good examples?

I am currently responsible for putting together a supplier assessment survey to submit to our critical suppliers. Does anyone know where I can see some good examples in order to get an idea on the best way to structure it?

Attached is a recently received example of a supplier survey. I received it as a hard copy document. As the one who gets to fill most of the bloody things out, I would prefer to receive them as emailed Excel or Word files. It makes them easier for me to file, keep track of, respond to and update as needed when I get the same form 2 years down the road from the same customer.

it is really very important subject you have here, because it could put the whole system in really bad situation.
any way, I have 2 questions
1- what is the difference between vendor and supplier
and if there is a difference, what will be the exact difference in their questionnaire or survey.

2- The criteria that could used to evaluate this supplier or vendor after receiving their feed back of the questionnaire?

1. A Supplier can be defined as a company which supplies parts or services to another company, also called vendor. (So basically they are interchangeable)

2. Evaluation is up to you really. Do they answer all the questions on the survey? Are the answers acceptable? Are they ISO certified? Do they have a Quality Manual? Is the Quality Manual kept up-to-date? All of these could be questions you are asking yourself as you review the survey/questionnaire.
