Thanks all for your valuable replies, according to me the root cause I have come across is :

Root cause :- Predictive MTN is not being held due to the negligence of Maintenance team (From Past 6 months), only Preventive Mtn is being scheduled at proper time

So for the negligence what should i show the proper reason to auditor and what correction & corrective action should i take for this ?? do suggest

What type of corrective action tools do you use? 8D with 3 Legged 5Why? I recommend it.

I think, part of the initial investigation should be to determine if any incorrect parts went to any customers, unknowingly.

I think the root cause of of not performing the PMs will boil down to someone not using the PM system in place, for whatever reason.

I also think the reason it wasn't discovered or the systems broke down will probably be an ineffective Management Review.

This is my guess of course.
We use 8D with 5 M fishbone diagram

I'm not sure that's the root cause. Can you still reasonably ask why? Why were they negligent? When you truly reach the root cause, the corrective action is usually very evident.

As for the correction, catch up on the predictive maintenance. Correction includes containment when appropriate, so determine if any bad product happened as a result of the unperformed maintenance, and if so, determine how to contain it.

Rahul,

Care comes from the leaders.

Your fishbones may have omitted Care as one of the important ingredients to an effective process.

If the workers don't care about the requirements then look at the leader (and, perhaps, the leader's colleagues).

Determine what caused the negligence.

Frankly to be speaking there's no predictive Mtn held at our works only Preventive & Breakdown is been held at our end so our Mtn leader records are pending,

But according to IATF my role is conduct audit and find out NC's, so now we are struck to find for proper root cause, correction & corrective action .

Are you saying that you have internal requirements to do predictive maintenance (as opposed to preventive maintenance) and it's not being done at all?

Rahul,

Your top management require predictive maintenance so kindly describe what have they done to implement predictive maintenance:

1. Is there a plan to implement predictive maintenance?
2. Is the predictive maintenance plan fully resourced?
3. Have they provided training to the machine operators and the maintenance team?
4. Have they provided the necessary equipment?
5. Have they monitoring the implementation of the plan and the effectiveness of the resulting predictive maintenance?
6. Or, are they seeking to enforce their requirement solely through you as the internal auditor?

Only after completing actions 1 through 5 above should you audit predictive maintenance otherwise you are an auditor trying to act as a manager. Auditors focus on how well the system provided by management helps the employees to fulfill management requirements. As auditor you cannot do management’s job.

Your reference to IATF suggests that top management wants to qualify for automotive work. If so your question is best addressed under the IATF 16949 forum instead of ISO 9001.

Let me give you a link:

IATF 16949 Section 8.5.1.5 (Total Productive Maintenance)

Best wishes,

John

IATF? Is your company registered to 16949? This is a 9001 forum. We may not be giving you correct advice.
